12th Grade Students Immunizations

Parents of 12th Grade students:

This is to inform you of the Ohio Revised Code regarding required immunizations for school attendance for the school year. 

All students entering 12th grade  are now required by the state of Ohio to show proof of the following immunizations PRIOR to starting school:

Meningococcal Vaccine,  MCV 4, (protection from Meningitis)

If your child received a Meningitis vaccine before the age of 16, a second dose is required.

If your child received the first dose of Meningitis vaccine and is 16 years or older, only the one dose is required to attend school in the 12th grade.

This vaccine can be obtained from now until the first day of 12th grade.  Please contact your primary care physician, the Columbiana County Health Department, 330-424-0272, or the East Liverpool City Health Department, 330-385-5123, for an appointment.

Please send in a copy of the immunization record or have it faxed to, 330-679-3005, as soon as it is obtained.  If the immunization record is not on file your child can be excluded from starting school.  Contact me for any concerns or questions at 330-679-2301, option 5 for school nurse.

7th Grade Students Immunizations

Attention  Parents/Guardians of:   7th Grade Students for 2019-2020 School Year:
 
This is to inform you of the Ohio Revised Code regarding required immunizations for school attendance for the school year.  Students will be required to have two immunizations.

One dose of TDap (Tetanus, Diphtheria, and Acellular Pertussis, adolescent and adult formulation) or Td (Tetanus and Diphtheria, Adult ) vaccine shall be required PRIOR to entry into the seventh (7th) grade.  This is required from the Ohio Revised Code Section 3701.13.   This dose is intended to be administered as a booster dose for students who have completed the required dose of the initial series of DTaP/DT/Td vaccine.

 One dose of Meningococcal Vaccine (Serogroup A,C,W, and Y) to protect from Meningitis, is also required PRIOR to school next year.   This is a new requirement starting from 2016 school year.

Please contact your child’s physician,  the Columbiana County Health Department at 330-424-0272,  or the East Liverpool City Health Department 330-385-5123 as soon as possible to schedule an appointment over the summer.  

 I need to have a copy of the vaccines before school starts in August.  Your child may be excluded from starting school if they are not on file.
